Certificate of Service
Section 42 of the Basic Conditions of Employment Act obliges you to give every departing employee a certificate of service, and it prescribes what goes on it. This covers all of it: name, position, dates engaged and terminated, and remuneration on termination. The reason for leaving sits at the bottom because the Act only requires it when the employee asks.
